Help, support, and social opportunities for people going through the cancer journey, and their friends and families. FACT offers support outside the hospital environment for everyone affected by cancer. No referral is necessary and both patients and non patients can benefit from FACT's services.

The Primary Care Network (PCN) Cancer Care Team
The Cancer Care Coordinators are available to all practices and patients across South Tyneside. Services include:
Supporting:
all patients with a new diagnosis of cancer.
vulnerable patients at risk of not attending a fast track suspected cancer appointment.
Providing non‑medical support to patients working closely with the general practitioner and other health and social professionals to ensure that the patients are accessing the right care and support.
If you are worried about a patient not attending their fast track suspected cancer appointment or they have previously not attended, request support for the patient by sending an email to southtyneside.pcncancercareteam@nhs.net.
The Cancer Care Coordination service offers an open door policy to patients but with a minimum of three touch points:
An introductory new cancer diagnosis telephone call to the patient to:
Explain their role.
Answer any immediate questions.
Signpost.
Offering patients the opportunity for a discussion and informing them of the support available from primary care, within 3 months of diagnosis.
Offering patients with cancer, a cancer care review using a structured template recorded as occurring within 12 months of the date of diagnosis.
