Patient Registration

REGISTER AS A NEW PATIENT

If you live in the post code areas SR6 and NE36 you can register with our practice. You can register with us online by clicking the “click here to register Online” button above. All new patients will receive a new patient health check when they register.

You will need to complete a New Patient  registration form and a new patient questionnaire – this will provide us with useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients will receive a new patient health check when they register.

Please Note:

  • Registration with the practice will commence when you attend your appointment with our HCA
  • When you come to see our HCA, you must remember to bring along your completed Registration form (if you have not registered online), New Patient Questionnaire and signed Practice Prescribing Agreement forms.
  • Practice Prescribing Agreement. Some categories of medicine may pose particular risks of harm o r may be associated with misuse, overuse or addiction and its practice policy to minimize the use of medicines associated with dependence.

TEMPORARY REGISTRATION

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot however register as a temporary patient at a practice in the town or area where you are already registered.
